There are two steps to follow:

1. FILL OUT PAYMENT REQUEST FORM and we will send you a link to pay with credit card.

2. FILL OUT THE ONLINE REGISTRATION FORM (This will be emailed to you after your payment is made)

We will confirm with you after you have completed everything!
